Base 25 - Service Experience Questionnaire - (12-18+ years old)

Overview

This questionnaire is for Young People aged 12 - 18 years and beyond, who have accessed a Base 25 service from the year 2021 up to the present day.

We would like to hear your views and feelings about Base 25's Services. It will help City of Wolverhampton Council and NHS CAMHS understand how these services are meeting young people's needs. Your answers will be completely anonymous (no one will know these responses are yours).
