Base 25 - Service Experience Questionnaire - (Parents and carers)

Overview

This questionnaire is for parents and carers of young people who have accessed a Base 25 service since 2021.

We would like to hear your views and feelings about Base 25's services. We feel it is important to understand the impact this service has had on young people and their families. It will help City of Wolverhampton Council and CAMHS understand how these services are meeting young people's needs.
