2-Year Developmental Review: Parent/Carer Feedback Survey

Overview

This survey aims to understand parent/carer experiences of their child’s 2-year review appointment.

Why your views matter

We want to hear from parents/carers who have recently (in the past 6 months) attended a 2-year review. Your feedback will help us understand how easy it is to access this appointment, what works well, and where improvements can be made.

This short survey asks about:

  • How easy it was to attend (e.g. travel, timing, accessibility)
  • How useful you found the review
  • Your perceptions on the value and importance of the review
  • Whether you had any concerns or questions going into the appointment
  • Your preferred way of being contacted
  • Any suggestions for improving access/ making the experience better

Responses are anonymous and should take around 5-10 minutes to complete.

Upon completing the survey, you will be eligible for one £5 ASDA voucher, subject to availability. Vouchers can be collected from your local Family Hub with proof of survey completion.

Please note that translated copies of the survey are available in 4 languages (Polish, Arabic, Punjabi and Arabic). Please speak to a member of staff at the Family Hub Reception desk if will you need a translated copy.
